Looks and fits like the old part, but with no identifying info. I thought this module had failed, but it turned out that the fan connectors were not seated fully, and vibrated loose after a couple of weeks. When I clicked the fan connectors securely onto the module, they started working again. If they vibrate loose again, I'll know that something doesn't fit right. (The fan connectors to the fan are undamaged -- they still have their click lock part.)
So far, so good. [July 2021]

Fits 06 Mazda 6 V6 3.0L. Fans would constantly run at full rpm as soon as you turned the key to on then while driving and would not stop going full send until after you removed the key and about 15 seconds later it would finally shut off. Priced this on rock auto and was $65 for a Mazda but turns out Mitsubishi uses the same one for a fraction of the price. Works perfect easy install and the noise from speedy fans is gone. they rarely ever kick on but yes do kick on and work.10/10
