I tried this on my little toe. Did not try the inscerts. Found it was easier to roll up a peice of toilet paper and put it between my fourth and little toe. This is not a bad product and might work better for others.

I'd never attempt to wear these in shoes - barely fit in a slide sandal. I wear them after a shower from work and the relief is amazing - still tender on the 5th but is better than rub on pain relief after walking 5-10 miles a day, 5 days a week.
Fit toe spacer on before using the shim.

Cannot speak to anyone else's experience, but for me this product was a godsend. Fits perfectly and very comfortable. Most pain relieved immediately, now gone, and I had a very large, red, painful tailor's bunion that is visibly 80% less protruded and red inside of 3 days.
I kept it on all day and took it off at night. It will feel a little weird while it is adjusting the foot but that is how these things work. Make sure you wear shoes that are wide enough so that your feet are not scrunched in - probably will still get a similar result but this is the most comfortable route.
I wear size 10.5 EEE shoes, bought the large, and wear it with the stint in it.
