On the box ends: "12-1076 Baseline Large Joint Goniometer" and "Baseline Evaluation Instruments". Box face and back lists both Fabrication Enterprises of White Plains, NY and RMS UK Lt. Sommerset UK as manufacturer/distributors. UPC 714905001023. Sticker on the box face gives a "vendor SKU #" of 12-1076 along with a different UPC of 714905015020 along with "CCO" (country of origin) China.
Product measures 18" long and 11-3/4 wide. The clear plexiglas is approximately 5/32" (4mm) thick and is generally rigid but you can flex the material slightly. The half-moon cutout is centered on the sheet going half the width in (about 6") and leaving about 5" at the 0/90 marks on either side.
The degree markings are large and easy to read. The little 5/8" long level is glued into a notch at the center 0 degree. I noted that the level slightly thicker than the plexiglas sheet and is offset slightly so that it is above the "face". This means if you set it upside down and pressure is put on the level it could be knocked off. Probably not an issue if you hang it or set it on edge to store it. The lines and numbers appear to be painted on rather than decals (stickers) which likely will add to the life of the product.
If it wasn't marketed as a medical device it would probably be half the price. Generally well made and will serve its purpose.
