I have been dealing with a L5 disc issue this year, and being the owner of a landscaping company who mows 8 hours a day, I could not find an iceing option that would fit my needs. I had to either deal with ice in plastic bags and get a very wet butt, or sit on hard/lumpy ice packs made for my beverage cooler, or deal with traditional ice packs that lost the coldness in 15 minutes. Then I Googled for options for my ice pack needs. This has drastically improved my iceing needs. These really do what they say, stay cold for a long period of time. I like the fact it folds to save freezer space. It gets very cold so I put it in a flannel pillowcase that I have leave one layer or two between my back and the ice pack. I have three now.

This ice pack is pretty awesome. As soon as you get it out of the box you can see the good quality of it. And they are cold, I mean COLD. Definitely follow the manufacturer's instructions. The second time I used it I placed it directly on my skin for almost an hour (two things you are not supposed to do) and it actually froze a small fist sized section of my skin solid. It felt like there was a plastic disc inserted right underneath my top skin layer. So ya, they are that cold.

This really is the coldest ice pack I've ever used. I bought the small and larger one, but I have returned the larger one because I mistakenly thought that it could be used as either hot or cold therapy. The smaller one does both, which I need. Otherwise, very happy so far with the product.
