Ignore the poor reviews it needs no instruction it is common sense. The strap does need to be extended...easily done. The cup has a valve on top use it to create a suction. My father can finally sleep through the night this is a God send!

Back flows.....a lot

So the fabric is easy to tear and the bag can let urine flow in both directions freely. Come to find out if you break the underwear that holds the part your penis sits in this becomes near unusable. It's loose around the penis and can fall off fairly easily. If you're lying down and find yourself urinating you may end up having it just go all over you and your bed..... worst design ever
As for the back flow issue, I set the external catheter part down while trying to fix the underwear and got urine all over my floor. Not because I peed but because the bag allowed urine to flow back up the tube and out of the catheter piece. I got a different external catheter that is way more comfortable and doesn't flow backwards for a bit more. I don't recommend this one
