when I first received it (very fast delivery) I had ny doubts as to its usefulness. I am 6 ft 2 and weigh 228 lbs and developed upper body but not exceptional. The pack has difficulty sliding up high enough to cover the shoulder. I will be having a total shoulder replacement. Before sending it back I thought I would give it a dry run prior to surgery using the ice packs. The problem is not with the diameter of the cuff area but the cuff area is not cut deep enough to enable the main cooling area to slide all the way over the shoulder area before hitting the armpit. A better design would have been to have the section connecting the cuff to itself to be made out of a stretchable material. That would enable the entire pack to easily slide up further on the shoulder. As it now stands with the help of my wife to pull the pack up further it serves the purpose and cools the area of where the incision will be. But one cannot put this on by oneself. Definitely need help. This is the x-large unit and if you are of smaller frame maybe able to do it alone. Ice packs are great and very flexible. I rated it in some instances with 3-4 stars since I have not used it after surgery. Will add more in 3 days after surgery.

Update: I'm leaving my initial review in the hopes that others with this issue will know how to fix it and how awesome the customer service is with this company. After leaving this review, I got a message next day telling me that they would refund my purchase price and to keep the wrap to give to someone else who might be able to use it. While I believe the item I received is a defective item (the arm hole and shoulder sections are a clear mismatch), the cold packs are the bomb, and I will definitely use them! Plus, they refunded my money as well as allowing me to keep the item. I may try to make an adjustment to the arm to see if I can make the product usable. Regardless, I give their customer service great marks and I would try one of their products again for that reason alone. Just know you need to contact CS if you have an issue and they will work it out promptly. Amazon won't allow returns through their system.

I love the gel packs that come with this product. I liked that the product had multiple ice packs in it and that they were made of hot/cold gel. I was so excited to try it. As a plus sized woman, I ordered the XL size to ensure it would fit around my arms. My arms aren't huge, but they're not small either. So it's a usual concern for me. I wear about a size 16/18 in tops and dresses. Many customers had noted how large the l/XL wrap was, so I was sure it would be big if anything. The armhole on this wrap was so tight, I couldn't wear it comfortably for even a few minutes. Additionally, the shoulder portion was so much bigger than my chest that I couldn't make it fit by tightening the straps because my arm wouldn't fit all the way into the sleeve. There is no way a man with large muscles would have been able to wear this shoulder wrap. Is it a fluke? A defect? I have no idea because the worst part of the entire experience is that I specifically ordered this wrap because the description says it's refundable and backed by a fit guarantee. But when you try to return it, Amazon says it is ineligible for return. Nice bait and switch! So, I guess I'm out the cost of this wrap because I can't wear it and can't return it. Total waste. Buyer beware. If you're concerned about size, look elsewhere!

This is our second purchase of this shoulder wrap and the first one lasted many years. Really works great for anyone with shoulder pain or after shoulder surgery for pain.

Strap opposite the Rotator cuff wrap Digs into your body. A wide easment is needed in the middle of it for padding. Otherwise this is fabulous. I'm a little on the heavy side so the x-large isn't quite large enough, but boy does the heat and cold feel wonderful. I'll trade the rotator cuff pain for the digging discomfort. 4-22 Item was cutting off circulation had to order another BRAND to get a 3X with 20 bicep.

Made holding the ice pack easy. No more uncomfortable bulky ice packs. Nice pockets to slide ice pads in. Can use ice on both sides of shoulders at once hands free.
